Clinical Programming Assessment and Training

Assessment and training for current and proposed regulatory requirements.  It doesn't matter how long we are in this industry, there are misperceptions and confusion related to current regulations let alone the consistent introduction of new regulations and changes being imposed.

Effective transitions of care across the multiple settings; acute>inpatient rehab>long term care hospitals>skilled nursing>home health>outpatient is CRITICAL to assuring quality care for our residents/patients, as well as achieving all aspects of healthcare reform and the Triple Aim.  Understanding how to optimize data in order to obtain UPSTREAM and DOWNSTREAM partnerships can be key in creating referral sources and establishing narrowed networks based on quality care and clinical outcomes.

In preparation for the new payment model, PDPM, slated for FY2020, identifying opportunities for efficiency and effectiveness in post-acute care coordination through a thorough analysis of operational and clinical strategies, policies and procedures and customer service is a must!  A deep dive into care planning, modes of service delivery, staff/skill mix, interdisciplinary communication, review of QMs and Pepper Reports, and discharge planning will be reviewed, along with discussions and observations will be used to determine potential opportunities for improvement or change.

​​​Clinical programming can include skilled therapy, restorative nursing or nursing maintenance programs that are designed to ensure the patients maintain their highest practicable level of function and when appropriate are effectively and efficiently discharged to the next necessary level of care.
