It was established to help you accomplish your goals through a variety of services including but not limited to:

• How to establish upstream and downstream partnerships, what to know, what to ask and what to provide

Care Navigation Consulting wants to provide you SOLUTIONS!!  My mission is to provide you with the information you need to effectively and efficiently navigate patients through the care continuum!  As a Physical Therapist who has worked in the post-acute settings for over 25 years, I have a passion for our Seniors and an appreciation for the caregivers doing everything they can do to make the experience positive and successful.  I believe it is possible to provide what the patient needs and do it in a way that allows providers to be sustainable.  I have always lived by the statement, “if you do right by the patient, the rest will follow.”  While many aspects of our industry have changed, I still believe in this statement and will work to make it happen with you and your patients or clients!
